Responsibilities:
Provide home health and hospice services to individuals, their families Collaborate with the multidisciplinary group and other caregivers to meet specific goals Assures access to other disciplines, as needed.
Requirements:
Current RN licensure in the State of Utah Current Utah driver's license and reliable transportation Excellent communication skills In home patient visits Medicine management Wound care IV skills Full-Time
